ACKNOWLEDGMENT
You, the Petitioner, will be required to sign,
and have Notarized, a
formal legal document acknowledging that you have been advised by your Attorney,
and that you understand, recognize, and agree with the following integral
components of our professional relationship, by which you will abide in exchange
for the Office accepting your case:

Timothy Kelly & Associates, LLC to represent me and I am the Client, if
the Office should attain positive results by finding the person(s) for whom I
am searching but a negative response because they decline to consent to any
further contact with or from me, both I and the Office must and will honor
their decision and respect their wishes not to be identified to me.

| I understand that this means that I will not,
under any circumstances, be provided with any information to place myself in
contact with them unless they should agree to such IN WRITING and the Court
issues an Order Authorizing Release of such Information to me |
| Furthermore, pursuant to this Acknowledgment,
I understand that in the event that the person(s) for whom I search are
located but decline further contact from me, I am not and will not become
entitled, by virtue of our Attorney-Client relationship, and do hereby waive
such right, to access to any identifying information in my Attorney's, or the
Office's possession or control, or in the possession or control of its
agent/employee, the Confidential Intermediary. |

of a notary public, and returned to the Office for filing with
the initial Petition. |
